What state boards expect on a massage-therapy invoice

Most US state massage boards require licensed therapists to maintain detailed treatment records and to provide clients with receipts that include the therapist's full name, license/registration number, date and duration of service, and modality. The exact requirements vary — California's CAMTC requirements differ from New York's, Florida's, and Texas's — but the spirit is the same: the client and any auditing body should be able to identify the therapist, the date, and the work performed.
