Childcare Receipt Template
Form 2441 Ready · Tax Credit Compliant
Parents claiming the Child and Dependent Care Credit (or submitting to a dependent-care FSA) need receipts that include the caregiver's tax ID, the qualifying child's name, the period covered, and the amount paid. Issueable's childcare receipt format includes every required field, so the same document works for IRS Form 2441 and FSA reimbursement.

Qualifying Child Detail
Child's name and date of birth fields document the qualifying dependent for tax-credit eligibility.
Period & Hours
Date range covered with hours of care broken out — works for both annual tax filings and monthly/quarterly FSA substantiation.

Family Daycare Providers

Childcare Receipt FAQ
Why parents care about complete receipts
The Child and Dependent Care Credit can return up to $1,050 per child (and $2,100 for two or more) to qualifying parents — but only if the IRS-required substantiation is in place. Parents who can't produce the caregiver's tax ID at filing time lose the credit entirely. For dependent-care FSA participants, missing substantiation means the FSA administrator denies reimbursement and the parent loses the contribution.
Childcare providers who issue complete receipts proactively (rather than fielding ad-hoc tax-time requests in January) win parent loyalty and avoid the year-end scramble. A monthly receipt that's already compliant doubles as the annual substantiation; the parent forwards it to their tax preparer and you don't get a phone call in April.
